ALL CLIENTS ARE REQUIRED TO COMPLETE AND SIGN THE NECESSARY FORMS PRIOR TO RECEIVING SERVICES.
The forms required are in your best interest regarding potential concerns or risks, such as contraindications, that may interfere with your results and that may damage your skin during your ritual/facial | sugaring | lash services. As always, your safety is first and foremost.

STUDIO WELLNESS GUIDELINES + COVID 19 LIABILITY WAIVER | Due to the nature of close contact one-on-one services, and the safety for clients, community and myself the COVID-19 Form is required and must be completed prior to your first visit and every January.
FACIAL FORMS
CLIENT CONSULTATION | You are required to complete and sign the New Client Holistic Ritual | Facial Intake Form below, to ensure Ritual Skincare Sugaring + Sound can answer questions, discuss concerns + expectations and options for ritual/facial or products. If this form is not completed prior to your appointment time, you will be required to do so when you arrive. This will cut into your service time and the service may need to be modified.
NEW CLIENTS | You are required to complete and sign the New Client Holistic Ritual | Facial Intake Form below, to ensure Ritual Skincare Sugaring + Sound can create the most beneficial treatment for you. Your skin and body health history is required, since contraindications may apply with products or modalities. If this form is not completed prior to your appointment time, you will be required to do so when you arrive. This will cut into your service time, with no price adjustments and the service may need to be modified.
ESTABLISHED CLIENTS | You are required to complete and sign the Established Client Holistic Ritual | Facial Intake Form below, before EVERY appointment to ensure Ritual Skincare Sugaring + Sound can create the most beneficial treatment for you. Ritual Skincare Sugaring + Sound needs to be aware of changes regarding your skin and body health history, since contraindications may apply with products or modalities. If this form is not completed before your appointment time, you will be required to do so when you arrive. This will cut into your service time, with no price adjustments.
SUGARING FORMS
NEW CLIENTS | You are required to complete and sign The New Client Sugaring Intake Form prior to your first appointment, to ensure there are not any contraindications that may interfere with the results or that may damage your skin during your service. If this form is not completed before your appointment time, you will be required to do so when you arrive. This will cut into your service time, with no price adjustments and the service may need to be modified.
ESTABLISHED CLIENTS | It is absolutely necessary and you are required to update Ritual Skincare Sugaring + Sound regarding changes to your medications (oral, topical products or treatments), skin and body health history. This is in your best interest, to ensure there are not any contraindications with products, modalities or sugar paste, that may interfere with your results and damage your skin during your service. Established clients are required to complete the Established Client Update Sugaring Intake Form, below, in January.
LASH LIFT FORMS
NEW + ESTABLISHED CLIENTS | You are required to complete and sign the Lash Lift Intake Form below, before EVERY appointment to ensure Ritual Skincare Sugaring + Sound can create the most beneficial treatment for you. Ritual Skincare Sugaring + Sound needs to be aware of changes regarding your skin and body health history, since contraindications may apply. If this form is not completed before your appointment time, you will be required to do so when you arrive. This will cut into your service time, with no price adjustments and the service may need to be modified.

Curate your perfect Wish List with the services, products or gift certificate denomination you would love to indulge in. Build it... share it... get exactly what you want! This will make your experience of receiving what you want more enjoyable and easier for those shopping for you.

Please know that if you are interested in sugaring services, I will not discuss the sugaring service(s) you are interested in receiving, with the person purchasing the gift for you. I will inform them of the amount of the service(s), so they can purchase a gift certificate for you.

The form you complete is your choice. Some people prefer to have everything in one form that’s easy to click on and others prefer to look at the website and type their list.
